Dear parents,

we are confidently asking for your favor in the form of a completed declaration of honour to register your child as a student of SZUŠ Ružová dolina 29, Bratislava or SZUŠ Bučany 155.

The declaration of honour of the pupil's legal representative (in the case of the age of majority of the pupil) has a decisive influence on the provision of teaching, its quality and also on the improvement of the material and technical equipment of the school.

On the basis of this statement, each pupil will be included in the collection of data, which serves as a basis for the purpose of allocating funds for schools for the following calendar year.

If a pupil of a primary art school attends more than one school of the same kind (for example, attends both public and private or church ZUŠ), the pupil's legal representative (in the case of adulthood, the pupil himself) will provide a written declaration to only one relevant school.

The form also includes the consent of the legal representative (in the case of the adult of the student himself) GDPR. Consent to the processing of data will allow us to handle your personal data to the extent strictly necessary.
